Dessaturação de oxigênio induzida pelo exercício e características do sono noturno em indivíduos com DPOC

| dc.description.abstract | Introdução: A dessaturação de oxigênio induzida pelo exercício (DIE) é relativamente comum em indivíduos com doença pulmonar obstrutiva crônica (DPOC) e já foi associada à rigidez arterial, baixa capacidade de exercício e mortalidade nessa população. Outros fatores reconhecidamente associados à DIE são a obesidade, limitação ao fluxo aéreo e baixa saturação de oxigênio (SpO2) em repouso. Sabe-se também que indivíduos com DPOC frequentemente apresentam distúrbios do sono. No entanto, ainda não se sabe se a DIE está associada às características do sono nesta população. Objetivo: Verificar se há associação das variáveis respiratórias poligráficas do sono noturno com a DIE durante testes simples de força muscular e de capacidade funcional de exercício em indivíduos com DPOC. Métodos: Neste estudo transversal, indivíduos com DPOC foram submetidos à avaliação da função pulmonar pela espirometria e registro contínuo da SpO2 por um oxímetro de pulso durante os seguintes testes: contração isométrica voluntária máxima de quadríceps femoral dominante (força de QF) e os testes funcionais Sit-to-Stand (STS), Timed Up and Go (TUG) e Four-Meter Gait Speed (4MGS). Os distúrbios respiratórios do sono foram avaliados por poligrafia (ApneaLinkTM Air, Resmed, Austrália) por uma noite. De acordo com a ocorrência de SpO2 abaixo do ponto de corte (i.e., <90%) durante os testes, os indivíduos foram alocados nos grupos com DIE e sem DIE. Para análise estatística, o teste de Shapiro-Wilk foi utilizado para verificar a normalidade dos dados, que posteriormente foram comparados pelo Teste T ou de Mann-Whitney. Correlações foram avaliadas pelo coeficiente de Pearson, e foi realizada regressão logística para investigar em mais profundidade a associação da ocorrência de DIE como variável dependente e outras variáveis independentes. Foi utilizado o software SPSS (20.0) e adotada significância estatística de p<0,05. Resultados: Foram avaliados 28 indivíduos com DPOC (69±7 anos; IMC: 26±5 Kg/m2; índice de apneia e hipopneia 14±8). Na comparação entre os grupos com e sem DIE, o grupo com DIE no teste de força de QF apresentava maior idade (73±5 vs 66 ±7 anos; p=0,029), enquanto o grupo com DIE nos testes TUG e STS apresentava pior VEF1 (respectivamente 44 [31-58] vs 64 (50-77) %; p=0,013 e 45±13 vs 65±16 %; p=0,012). Com relação às características do sono, o grupo com DIE no teste de força de QF apresentava menor SpO2 média e maior tempo em SpO2 <85% e <80%. O grupo com DIE no TUG apresentava menor SpO2 média e basal. O grupo com DIE no STS apresentava menor SpO2 média, mínima e basal, além de maior tempo em SpO2 <85% e <80%. Foram observadas correlações consistentes e significantes de diversas variáveis da poligrafia com a SpO2 mínima atingida nos quatro testes, principalmente no STS. A regressão logística mostrou associação significativa da probabilidade de ocorrência de DIE em diferentes testes com SpO2 média mais baixa durante a poligrafia, maior idade, pior VEF1 e SpO2 mais baixa ao repouso. Conclusão: Indivíduos com DPOC que dessaturam em diferentes testes físicos apresentam piores desfechos respiratórios durante o sono noturno em comparação àqueles que não dessaturam nos testes. SpO2 média mais baixa durante a poligrafia, maior idade, pior VEF1 e SpO2 mais baixa ao repouso se associaram significativamente à probabilidade de ocorrência da DIE em diferentes testes. De forma geral, o teste que melhor evidenciou a associação entre DIE e dessaturação noturna foi o STS. Portanto, a aplicação nessa população de testes simples de força muscular e de capacidade funcional de exercício, em especial o STS, ajuda na identificação de indivíduos com DIE que apresentam piores desfechos respiratórios durante o sono noturno | |
| dc.description.abstractother1 | Introduction: Exercise-induced oxygen desaturation (EID) is relatively common in individuals with chronic obstructive pulmonary disease (COPD) and has been associated with arterial stiffness, reduced exercise capacity, and increased mortality in this population. Other well-known factors associated with EID include obesity, airflow limitation, and low resting oxygen saturation (SpO2). It is also known that individuals with COPD frequently experience sleep disturbances. However, it remains unclear whether EID is associated with sleep characteristics in this population. Objective: To investigate whether nocturnal sleep polygraphic variables are associated with EID during simple muscle strength and functional exercise capacity tests in individuals with COPD. Methods: In this cross-sectional study, individuals with COPD underwent pulmonary function assessment by spirometry and continuous SpO2 monitoring using a pulse oximeter during the following tests: maximal voluntary isometric contraction of the dominant quadriceps femoris (QF strength), and the functional tests Sit-to-Stand (STS), Timed Up and Go (TUG), and Four-Meter Gait Speed (4MGS). Sleep-disordered breathing was assessed through overnight polygraphy (ApneaLink™ Air, ResMed, Australia). Based on the occurrence of SpO2 falling below the cutoff point (<90%) during the tests, participants were divided into groups with and without EID. The Shapiro-Wilk test was used to assess data normality, followed by comparisons using the Student’s t-test or Mann-Whitney test. Pearson’s coefficient was used to analyze correlations, and logistic regression was performed to further investigate association between the occurrence of EID as dependent variable and other independent variables. Statistical analyses were conducted using SPSS version 20.0, with significance set at p<0.05. Results: Twenty-eight individuals with COPD were evaluated (69±7 years; BMI: 26±5 kg/m²; apnea-hypopnea index: 14±8). In the comparison between groups with and without EID, the group with EID during the QF strength test was older (73±5 vs. 66±7 years; p=0.029), while those with EID in the TUG and STS tests showed worse FEV1 (44 [31–58] vs. 64 [50–77] %; p=0.013 and 45±13 vs. 65±16 %; p=0.012, respectively). Regarding sleep characteristics, the group with EID during the QF strength test had lower mean SpO2 and spent more time with SpO2 <85% and <80%. The group with EID in the TUG test showed lower mean and baseline SpO2. The group with EID in the STS test showed lower mean, minimum, and baseline SpO2, along with increased time with SpO2 <85% and <80%. Significant and consistent correlations were found between various polygraphic variables and the minimum SpO2 reached during the four tests, particularly the STS. Logistic regression showed significant association of the probability to present EID in different tests with lower average SpO2 during polygraphy, higher age, worse FEV1 and lower SpO2 at rest. Conclusion: Individuals with COPD who experience desaturation during different physical tests have worse respiratory outcomes during sleep compared to those who do not desaturate. Lower average SpO2 during polygraphy, higher age, worse FEV1 and lower SpO2 at rest are significantly associated to the probability of occurring EID in different tests. Overall, the test that most clearly demonstrated the association between EID and nocturnal desaturation was the STS. Therefore, the use of simple muscle strength and functional capacity tests—especially the STS—can help identify individuals with EID who present worse respiratory outcomes during sleep | |
