dc.description.abstractA formação da parentalidade diante de uma gestação gemelar é permeada por especificidades, levando em conta a necessidade de a família se haver com o cuidado de dois bebês, que embora fisicamente semelhantes, precisam ser reconhecidos em sua singularidade. Em vista disso, a descoberta de uma gravidez de múltiplos implica em sentimentos ambivalentes e exige uma reorganização da rotina familiar. As mudanças provocadas pela chegada de dois bebês são ainda maiores diante do parto prematuro e a hospitalização das crianças em Unidade de Terapia Intensiva Neonatal (UTIN) – situação recorrente no nascimento de gêmeos – de modo que a relação materno-infantil passa a ser atravessada pela equipe de saúde e podem surgir entraves no processo de vinculação mãe-bebê, uma vez que o bebê prematuro é pouco responsivo ao contato, há a presença constante dos profissionais de saúde e a possibilidade de óbito infantil, o que pode contribuir para a família não investir afetivamente seus bebês. Assim, é primordial refletir sobre as vivências emocionais das famílias de bebês gêmeos que passaram pela UTIN, para que se construam intervenções psicológicas atentas às especificadas dessa relação e possa, com isso, prevenir potenciais complicações na saúde materno-infantil. Tendo em vista o interesse por estudar a relação materno-infantil diante da gemelaridade e nascimento prematuro, o presente estudo tem por objetivo investigar as experiências emocionais de puérperas, mães de gêmeos prematuros internados em Unidade de Terapia Intensiva Neonatal (UTIN). A coleta de dados ocorreu através de atendimentos em psicoterapia breve, às puérperas com bebês internados na UTIN do Hospital Universitário da Universidade Estadual de Londrina. Os dados foram analisados a partir da construção de fatos clínicos psicanalíticos, sendo estabelecidas duas categorias de análise: luto no puerpério e sobreviverei à essa monstanha russa? Na primeira verificou-se que em mães de gêmeos prematuros, acrescido ao processo elaborativo esperado no puerpério, há a necessidade de lidar com o luto pela perda do ideal construindo em torno da maternidade e dos bebês durante a gestação, tendo em vista o nascimento prematuro e a presença do hospital como terceiro na relação com os lactentes. Ademais, em situações onde ocorre óbito de um dos bebês, pode o cogêmeo ser colocado no lugar do gêmeo perdido pela mãe e/ou pais. No segundo eixo de análise, foram discutidos os sentimentos vivenciados pelas mães de UTIN e pela própria pequisadora, os quais são marcados pela angústia diante da incerteza e instabilidade do ambiente hospitalar e o uso de defesas primitivas como a projeção da raiva na equipe de saúde. Tais fatos demandam uma escuta qualificada para que os conteúdos afetivos possam ser acolhidos, de modo que tanto o psicológo como os demais profissionais de saúde ao exercer a função de rêverie possam contribuir para que as experiências emocionais sejam ressignificadas e compreendidas.
dc.description.abstractother1The formation of parenthood in the face of a twin pregnancy is permeated by specificities, taking into account the need for the family to take care of two babies, who, although physically similar, need to be recognized in their uniqueness. In view of this, the discovery of a multiple pregnancy implies ambivalent feelings and requires a reorganization of the family routine. The changes caused by the arrival of two babies are even greater in the face of premature birth and the hospitalization of children in the Neonatal Intensive Care Unit (NICU) – a recurring situation in the birth of twins – so that the mother-child relationship becomes crossed by health team and obstacles may arise in the mother-baby bonding process, since the premature baby is not very responsive to contact, there is the constant presence of health professionals and the possibility of infant death, which can contribute to the family not being able to affectively invest your babies. Therefore, it is essential to reflect on the emotional experiences of families of twin babies who have passed through the NICU, so that psychological interventions can be constructed that are attentive to the specifics of this relationship and can, therefore, prevent potential complications in maternal and child health. Given the interest in studying the mother-child relationship in the face of twinning and premature birth, the present study aims to investigate the emotional experiences of postpartum women, mothers of premature twins admitted to the Neonatal Intensive Care Unit (NICU). Data collection occurred through brief psychotherapy sessions for postpartum women with babies admitted to the NICU of the University Hospital of the State University of Londrina. The data were analyzed based on the construction of psychoanalytic clinical facts, establishing two categories of analysis: mourning in the postpartum period and will I survive this Russian mountain? In the first one, it was found that in mothers of premature twins, in addition to the elaborative process expected in the puerperium, there is a need to deal with the mourning for the loss of the ideal building around motherhood and babies during pregnancy, taking into account premature birth. and the presence of the hospital as a third party in the relationship with infants. Furthermore, in situations where one of the babies dies, the co-twin can be placed in the place of the lost twin by the mother and/or parents. In the second axis of analysis, the feelings experienced by NICU mothers and by the researcher herself were discussed, which are marked by anguish in the face of the uncertainty and instability of the hospital environment and the use of primitive defenses such as projecting anger onto the health team. Such facts demand qualified listening so that the affective contents can be welcomed, so that both the psychologist and other health professionals, when exercising the role of reverie, can contribute to the emotional experiences being given new meaning and understood.
