Investigação de variantes genéticas nos genes CD28 e ICOS frente a infecção pelo HPV no desenvolvimento de lesões intraepiteliais cervicais e de câncer de colo de útero

| dc.description.abstract | A resposta imune tem papel crucial tanto no controle da infecção pelo Papilomavírus Humano (HPV), quanto na prevenção da progressão de lesões cervicais para o câncer cervical. Entre as moléculas envolvidas nessa resposta, destacam-se o grupo de diferenciação 28 (CD28) e o coestimulador induzível de células T (ICOS), que atuam na ativação de células T. O CD28 está expresso constitutivamente em células T, enquanto que ICOS é induzido após a ativação do receptor de células T (TCR). Durante a ativação de células T, a coestimulação por CD28 promove a produção de citocinas e a proliferação de células T, enquanto ICOS exerce uma função essencial na diferenciação e atuação, principalmente, das células T foliculares auxiliares (Tfh). Variantes de nucleotídeo único (SNVs) nos genes CD28 e ICOS podem influenciar na expressão gênica dos receptores em células T, impedindo a estimulação necessária para a ativação de células T e consequentemente prejudicando a resposta imune frente a infecção por HPV, progressão de lesões cervicais e o câncer cervical. Neste contexto, o objetivo deste estudo consistiu em avaliar a associação dos alelos, genótipos e haplótipos das SNVs rs3116496 de CD28 e rs4404254 de ICOS com a infecção pelo HPV e o desenvolvimento de lesões intraepiteliais cervicais e de câncer cervical em uma amostra da população feminina do sul do Brasil. O projeto foi aprovado pelo comitê de ética em pesquisa envolvendo seres humanos da Universidade Estadual de Londrina (CAAE 38937520.2.0000.5231). As amostras de sangue periférico e de secreção cervical foram utilizadas para a extração de DNA genômico. Foram utilizadas PCR quantitativa e convencional para a genotipagem das SNVs rs3116496 de CD28 e rs4405254 de ICOS utilizando sondas validadas e caracterização da presença de HPV, respectivamente. As combinações haplotípicas das SNVs de CD28 e ICOS foram inferidas pelo software PHASE e as análises estatísticas foram realizadas pelo software SPSS Statistics, adotando-se nível de significância de p<0,05. Foram incluídas 450 mulheres, agrupadas inicialmente de acordo com a presença (n=263) ou ausência (n=187) da infecção pelo HPV. As mulheres infectadas pelo HPV foram subdivididas em: sem lesão cervical (n=116), lesões intraepiteliais de baixo grau (LIEBG) (n=21), lesões intraepiteliais de alto grau (LIEAG) (n=61) e câncer cervical (n=65). Observou-se que as características sociodemográficas, comportamentais e sexuais mostraram associação significativa com a infecção por HPV e a presença de lesões cervicais. Mulheres portadoras dos genótipos TC (OR: 2,029; IC95%: 1,028-4,007; p=0,041) e TC+CC (OR: 2,085; IC95%: 1,089-3.989; p=0,027) do SNV rs3116496 de CD28 apresentaram maior chance de desenvolvimento de LIEAG. A análise do SNV rs4404254 de ICOS não apresentou associação significativa com a infecção por HPV e as lesões cervicais, assim como a análise dos haplótipos. Os resultados indicam que a SNV rs3116496 de CD28 está associado com o desenvolvimento de LIEAG em uma coorte de mulheres brasileira, sugerindo que está variante pode influenciar nos mecanismos imunogenéticos envolvidos na progressão de lesões cervicais para câncer, dependendo de cofatores individuais | |
| dc.description.abstractother1 | The immune response plays a crucial role both in controlling Human Papillomavirus (HPV) infection and in preventing the progression of cervical lesions to cervical cancer. Among the molecules involved in this response, cluster of differentiation 28 (CD28) and inducible T-cell co-stimulator (ICOS) stand out, as they act in T-cell activation. CD28 is constitutively expressed on T cells, whereas ICOS is induced following T-cell receptor (TCR) activation. During T-cell activation, CD28 co-stimulation promotes cytokine production and T-cell proliferation, while ICOS plays an essential role in the differentiation and function, mainly, of follicular helper T cells (Tfh). Single nucleotide variants (SNVs) in the CD28 and ICOS genes may influence receptor gene expression in T cells, impairing the stimulation required for T-cell activation and consequently compromising the immune response against HPV infection, progression of cervical lesions, and cervical cancer. In this context, the aim of this study was to evaluate the association of alleles, genotypes, and haplotypes of the SNVs rs3116496 in CD28 and rs4404254 in ICOS with HPV infection and the development of cervical intraepithelial lesions and cervical cancer in a sample of the female population from southern Brazil. The project was approved by the Research Ethics Committee involving human subjects of the State University of Londrina (CAAE 38937520.2.0000.5231). Peripheral blood and cervical secretion samples were used for genomic DNA extraction. Quantitative and conventional PCR were employed for genotyping of the SNVs rs3116496 in CD28 and rs4404254 in ICOS using validated probes, and for HPV detection, respectively. Haplotype combinations of CD28 and ICOS SNVs were inferred using the PHASE software, and statistical analyses were performed with SPSS Statistics, adopting a significance level of p<0.05. A total of 450 women were included, initially grouped according to the presence (n=263) or absence (n=187) of HPV infection. HPV-positive women were further subdivided into: no cervical lesion (n=116), low-grade squamous intraepithelial lesion (LSIL) (n=21), high-grade squamous intraepithelial lesion (HSIL) (n=61), and cervical cancer (n=65). Sociodemographic, behavioral, and sexual characteristics showed a significant association with HPV infection and the presence of cervical lesions. Women carrying the TC genotype (OR: 2.029; 95% CI: 1.028–4.007; p=0.041) and the TC+CC genotypes (OR: 2.085; 95% CI: 1.089–3.989; p=0.027) of the CD28 rs3116496 SNV showed a higher likelihood of developing HSIL. Analysis of the SNV rs4404254 in ICOS showed no significant association with HPV infection or cervical lesions, nor did the haplotype analysis. The results indicate that the SNV rs3116496 in CD28 is associated with the development of HSIL in a cohort of Brazilian women, suggesting that this variant may influence immunogenetic mechanisms involved in the progression of cervical lesions to cancer, depending on individual cofactors | |
