Implicações das variantes genéticas (rs822335 e rs4143815) do gene de PD-L1 na suscetibilidade e prognóstico do câncer de mama

dc.description.abstractO câncer de mama (CM) é a neoplasia maligna mais frequente em mulheres no mundo e pode ser classificado em ao menos quatro diferentes subtipos moleculares, sendo eles: luminal A (LA), luminal B (LB), HER2 superexpresso (HER2+) e triplo negativo (TN), com os subtipos LA e TN considerados de prognósticos opostos, sendo LA o CM de prognóstico mais favorável, e TN classificado entre os mais agressivos. Apesar dos mecanismos do sistema imune para a eliminação de tumores, as células tumorais possuem como característica evolutiva o desenvolvimento de mecanismos de evasão imunológica. Os principais alvos de modulação pelos tumores incluem as moléculas de checkpoint imunológico, como o ligante de morte programada 1 (PD-L1) e seu receptor, a proteína de morte programada 1 (PD-1). O eixo PD-L1/PD-1 é essencial para suprimir a atividade de células T, atuando na manutenção da tolerância imunológica e no controle da inflamação. A expressão e função de PD-L1 pode ser influenciada pela presença de variantes genéticas, como as variantes de nucleotídeo único (SNVs). O gene que codifica PD-L1 é chamado CD274, e possui cerca de 20 SNVs indexadas na literatura, incluindo rs4143815G>C e rs822335T>C. A SNV rs4143815G>C localiza-se na região 3’UTR de CD274, enquanto rs822335T>C se encontra na região promotora do gene. Ambas já se correlacionaram com maior expressão de PD-L1, tornando-se alvos promissores de estudo para a melhor compreensão dos mecanismos de modulação de expressão de PD-L1, e de sua possível correlação com a suscetibilidade e o prognóstico do CM. Por isso, o objetivo deste estudo foi avaliar a influência dos polimorfismos rs4143815G>C e rs822335T>C de CD274, e seus haplótipos, em pacientes com CM, e entre os subtipos moleculares de prognósticos opostos mais frequentes dentro do grupo de amostras selecionado, LA e TN. Participaram do estudo 490 mulheres, divididas em dois grandes grupos: controle (n=198) e câncer (n=292), com o grupo câncer sendo subdividido em LA (n=182) LB (n=36), HER2+ (n=16) e TN (n=58). Foram utilizadas amostras de DNA armazenadas em biorrepositório e a genotipagem das SNVs foi realizada por Reação em Cadeia da Polimerase em Tempo Real (qPCR). Apenas o polimorfismo rs822335T>C demonstrou associações significativas com os grupos estudados, sendo seu genótipo TT associado à maior suscetibilidade ao CMLA (OR: 2.091; IC 95%: 1.086-4.023; p=0.027) e seu genótipo CT associado à proteção para o CMTN (OR: 0.439; IC 95%: 0.231-0.832; p=0.012), quando comparadas ao grupo controle. Além disso, ao avaliar os modelos genéticos, o modelo dominante indicou que a presença do alelo T confere proteção para o desenvolvimento do CMTN (OR: 0.465; IC 95%: 0.254-0.851; p=0.013), e o modelo recessivo confirma o genótipo TT como fator de suscetibilidade para CMLA (OR: 2.319; IC 95%: 1.285-4.187; p=0.005). Na análise haplotípica, os resultados foram semelhantes: a presença do haplótipo C-G demonstra-se como fator protetor para CMLA (OR: 0.592; IC 95%: 0.361-0.971; p=0.038), e aumenta a suscetibilidade ao CMTN em aproximadamente 2,2 vezes (OR: 2.170; IC 95%: 1.151-4.092; p=0.017), enquanto a presença do haplótipo T-G diminui em 2,6 vezes a suscetibilidade ao CMTN (OR: 0.379; IC 95%: 0.185-0.778; p=0.008). Este é o primeiro trabalho a avaliar estas SNVs em conjunto no contexto do CM, e demonstra a relação entre o alelo C e o genótipo CC de rs822335T>C, correlacionados com a maior expressão de PD-L1 em células tumorais, com a suscetibilidade ao CMTN - tumores que possuem prognóstico desfavorável e são muito imunogênicos, podendo ser favorecidos pelo aumento desta molécula no microambiente tumoral. Em adição, demonstramos a associação do alelo T e do genótipo TT, com o CMLA. Estes resultados podem contribuir para o melhor entendimento do papel de PD-L1 no CM.
dc.description.abstractother1Breast cancer (BC) is the most frequent malignant neoplasm in women worldwide and can be classified into at least four different molecular subtypes, namely: luminal A (LA), luminal B (LB), HER2-enriched (HER2+), and triple-negative (TN), with the LA and TN subtypes considered to have opposite prognoses — LA being the BC subtype with the most favorable prognosis, and TN classified among the most aggressive. Despite the immune system mechanisms for tumor elimination, tumor cells have evolved mechanisms for immune evasion. The main targets modulated by tumors include immune checkpoint molecules, such as programmed death-ligand 1 (PD-L1) and its receptor, programmed cell death protein 1 (PD-1). The PD-L1/PD-1 axis is essential for suppressing T cell activity, playing a role in maintaining immune tolerance and controlling inflammation. The expression and function of PD-L1 can be influenced by the presence of genetic variants, such as single nucleotide variants (SNVs). The gene encoding PD-L1 is called CD274, and about 20 SNVs are indexed in the literature, including rs4143815G>C and rs822335T>C. The SNV rs4143815G>C is located in the 3’UTR region of CD274, while rs822335T>C is found in an enhancer region within the gene promoter. Both have been correlated with higher PD-L1 expression, making them promising targets for study to better understand the mechanisms of PD-L1 expression modulation and their possible correlation with susceptibility and prognosis of BC. Therefore, the objective of this study was to evaluate the influence of the polymorphisms rs4143815G>C and rs822335T>C of CD274, and their haplotypes, in patients with and without BC, and among the most frequent molecular subtypes with opposite prognoses within the selected sample group, LA and TN. The study included 490 women divided into two major groups: control (n=198) and cancer (n=292), with the cancer group subdivided into LA (n=182), LB (n=36), HER2+ (n=16), and TN (n=58). DNA samples stored in a biorepository were used, and genotyping of the SNVs was performed by real-time polymerase chain reaction (qPCR). Only the polymorphism rs822335T>C showed significant associations with the studied groups, with the TT genotype associated with higher susceptibility to LABC (2.091; IC 95%: 1.086-4.023; p=0.027) and the CT genotype associated with protection against TNBC (OR: 0.439; IC 95%: 0.231-0.832; p=0.012), when compared to the control group. Furthermore, evaluating genetic models, the dominant model indicated that the presence of the T allele confers protection against the development of TNBC (OR: 0.465; IC 95%: 0.254-0.851; p=0.013), and the recessive model confirms the TT genotype as a susceptibility factor for LABC (OR: 2.319; IC 95%: 1.285-4.187; p=0.005). In the haplotypic analysis, results were similar: the presence of the C-G haplotype was protective against LABC (OR: 0.592; IC 95%: 0.361-0.971; p=0.038), and increased susceptibility to TNBC by 2.2 times (OR: 2.170; IC 95%: 1.151-4.092; p=0.017), whereas the presence of the T-G haplotype decreased susceptibility to TNBC by 2.6 times (OR: 0.379; IC 95%: 0.185-0.778; p=0.008). This is the first study to evaluate these SNVs jointly in the context of BC, demonstrating the relationship between the C allele and the CC genotype of rs822335T>C, correlated with higher PD-L1 expression in tumor cells, with susceptibility to TNBC - tumors that have a poor prognosis and are highly immunogenic, possibly favored by increased levels of this molecule in the tumor microenvironment. Additionally, we demonstrated the association of the T allele and TT genotype with the BC subtype with better prognosis. These results may contribute to a better understanding of the role of PD-L1 in BC.
