| dc.description.abstract | Resumo: O financiamento em saúde sempre foi um desafio para a gestão no SUS Nos últimos anos ocorreram algumas mudanças, como a aprovação da Emenda Constitucional 95, conhecida como a PEC do Teto dos gastos, e a mudança do financiamento da APS, com a aprovação e implantação do Programa Previne Brasil A pesquisa teve como objetivo analisar o processo de gestão orçamentária da APS em municípios da macrorregião norte do Paraná A macrorregião norte do Paraná, conta com uma população estimada de 1819461 habitantes distribuídos em 97 municípios A região é caracterizada por municípios de pequeno, médio e grande porte populacional O estudo ocorreu nos anos de 22, 221 e 222, em dois momentos, sendo o primeiro no formato quantitativo e o segundo de forma qualitativa Na primeira etapa, com o objetivo de investigar a evolução da composição tripartite para o financiamento dos serviços de saúde, realizou-se um estudo de natureza quantitativa, a partir de dados secundários, referentes ao financiamento dos municípios da macrorregião norte do Paraná Para a escolha dos locais de coleta no campo da pesquisa foi construída uma matriz de classificação dos municípios quanto à sua condição sobre o financiamento e a organização da APS Na segunda etapa, foram realizadas entrevistas a partir de um roteiro semiestruturado, tendo como sujeitos os contadores e gestores de saúde dos municípios elegíveis pela matriz de classificação Para análise das entrevistas utilizou-se o método de análise de discurso, preconizado por Martins e Bicudo (25), com o objetivo de analisar a organização do trabalho, planejamento em saúde e execução orçamentária referente aos investimentos para o custeio da APS, e as possíveis repercussões das recentes mudanças do financiamento, de forma qualitativa Os resultados da pesquisa mostraram que houve aumento da participação no financiamento pelos municípios, principalmente os de pequeno porte Mesmo com o aumento de recursos municipais investidos em saúde, os municípios menores apresentam maior dependência dos recursos de transferências constitucionais Em relação à organização do trabalho, planejamento em saúde e execução orçamentária referente aos investimentos para o custeio da atenção básica, a maioria dos municípios não apresenta uma equipe formal de construção de planejamento e orçamento; a construção do orçamento tende a ser mais coerente com o plano municipal de saúde quando existe uma equipe constituída, mesmo que informalmente; e a execução orçamentária também está intimamente relacionada com a maior capacidade técnica dessa equipe Os municípios menores, mostraram-se mais frágeis no que se refere à constituição de equipes mais qualificadas Sobre a percepção dos entrevistados sobre as mudanças nas regras de financiamento e sua possível implicação na gestão orçamentária, os municípios de grande porte reconhecem que houve diminuição dos recursos financeiros, principalmente os que possuem baixa cobertura da APS Observa-se que devido a pesquisa ter sido realizada em período pandêmico, houve repasse de recursos extraordinários para Covid, o que pode ter influenciado na percepção dos entrevistados sobre a diminuição dos recursos no âmbito da APS, principalmente em municípios menores O financiamento da APS e os valores investidos não acompanharam as necessidades de saúde apresentados no planejamento em saúde A insegurança em ter a garantia de recebimento pela União, a ausência dos Estados em incentivos financeiros no âmbito da APS e o medo de executar os recursos vinculados erroneamente, faz com que os municípios executem os gastos prioritariamente com recursos próprios | pt_BR |
| dc.description.abstractother1 | Abstract: Healthcare financing has always been a challenge to public healthcare system management In the last years there have been changes to it, such as the approval of Constitutional bill 95, known as Expense Limit act, and changes to the financing of APS with the implementation of the Brazil Prevention Program This research’s main objective was to analyze the process of APS budget management in the municipalities withing the northern macro region of Parana state This macro region accounts for an estimated population of 1819461 inhabitants distributed in 97 cities It is characterized by small, medium and large sized cities The study occurred along the years 22, 221 and 222, under two aspects, firstly in a quantitative format and secondly on a qualitative format In the first step, aiming to investigate the evolution of the triparty composition for public healthcare financing, a study of quantitative nature was realized, supported by secondary data, regarding the municipal financing of the northern macro region The choice of localities, where the research would take place, was built from a classification matrix with municipalities according to its conditions on financing and APS organization In the second step, interviews with a semi structured script were realized with accountants and healthcare managers of the eligible cities from the classification matrix To analyze the interview results it was used a method of standardized speeches by Martins & Bicudo (25) aiming to objectively analyze the work patterns, healthcare planning and budgetary execution regarding investments to support the APS, and its possible repercussions referring to financing changes, under a qualitative view The research demonstrates there has been an increase of financing participation by the municipalities, mainly for the smaller ones Even with the increases municipal resources invested in healthcare, the smaller cities show higher dependency of constitutionally transferred funds In regard to the labor organization, health planning and budgetary execution exclusive to investments and funding of basic healthcare attention, most of the cities do not have a formal structure to plan the assigned budget; the construction of such plan is more likely to be in line with municipal healthcare plan when there is a formal structure constituted, even if informal, and the budgetary execution is also intimately related with the technical capacity of such structure Smaller municipalities demonstrate to be more fragile in terms of constituting a qualified structure The perception of the interviewees about the financing changes and its implications to budget management, larges municipalities recognize there has been a reduction, mainly for the ones with lower coverage by the APS It must be observed that due to the research having happened within the COVID-19 pandemic period, extraordinary funds were handed over, which may have influenced the perception of the interviewees about reduction of resources under the APS, mainly in smaller cities The financing of the APS and values invested did not follow the healthcare needs as required by the healthcare planning The insecurity in having funds guaranteed by the Union, the absence of States to provide financial incentives under the APS, and the fear of wrongly executing the resources influenced the municipalities to prioritize own funds to execute the expenses | pt_BR |
