Análise das variantes genéticas rs1800896, rs1800871 e rs1800872 do gene IL10 e sua correlação com a suscetibilidade à infecção pelo HPV e desenvolvimento de câncer cervical

| dc.description.abstract | A interleucina 10 (IL-10) é uma citocina anti-inflamatória e imunossupressora que regula a resposta inflamatória e a imunidade celular contra infecções virais, incluindo o Papilomavírus Humano (HPV). Alterações na expressão e secreção da IL-10, potencialmente causadas por variantes de nucleotídeo único (SNVs) na região promotora do gene IL10, influenciam a persistência viral e a progressão para lesões intraepiteliais escamosas e câncer cervical. Este estudo teve como objetivo avaliar a associação dos alelos, genótipos e haplótipos das SNVs rs1800896 (-1082 A>G), rs1800871 (-819 C>T) e rs1800872 (-592 C>A) do gene IL10 com a infecção pelo HPV, o desenvolvimento de lesões intraepiteliais escamosas e câncer cervical em mulheres atendidas por serviços públicos de saúde no Paraná, Brasil. Trata-se de um estudo caso-controle, aprovado pelo Comitê de Ética em Pesquisa Envolvendo Seres Humanos da Universidade Estadual de Londrina (CAAE 38937520.2.0000.5231). Foram incluídas 438 mulheres no estudo, agrupadas de acordo com a presença (n=257) ou ausência (n=181) de infecção por HPV. O grupo infectado foi subdividido conforme o grau da lesão cervical: sem lesão (n=91), lesão intraepitelial escamosa de baixo grau (LIEBG) (n=25), lesão intraepitelial escamosa de alto grau (LIEAG) (n=72) e câncer cervical (n=69). Foram coletadas amostras de sangue periférico para genotipagem das variantes genéticas de IL10 por PCR em Tempo Real, e amostras cervicais para detecção de HPV por PCR convencional. As análises estatísticas foram conduzidas no software SPSS 25.0 (p<0,05), e a inferência haplotípica foi realizada pelo programa PHASE. Os resultados mostraram que o alelo G da variante rs1800896 esteve associado a um efeito protetor contra a infecção por HPV no modelo dominante (AA vs AG+GG; OR = 0,646, IC95%: 0,439–0,950; p = 0,026). Em contrapartida, o alelo T da rs1800871, no modelo dominante (CC vs CT+TT), esteve associado a maior suscetibilidade (OR = 2,056, IC95%: 1,388–3,045; p < 0,001), assim como os genótipos CA+AA da rs1800872 (OR = 2,056, IC95%: 1,388–3,045; p < 0,001). A análise haplotípica indicou que o haplótipo ATA esteve significativamente associado à infecção por HPV quando presente em mulheres homozigotas (ATA/ATA: OR = 2,238, IC95%: 1,155–4,339; p = 0,017) e em combinação com outros haplótipos (ATA/others: OR = 1,807, IC95%: 1,202–2,718; p = 0,004). Já o haplótipo GCC apresentou efeito protetor contra a infecção por HPV em mulheres heterozigotas para o haplótipo (GCC/others: OR = 0,628, IC95%: 0,420–0,940; p = 0,024). No entanto, após ajuste por fatores de confusão em modelos de regressão logística, essas associações com lesões e câncer não se mantiveram estatisticamente significativas (p>0,05). Este é o primeiro estudo a avaliar a associação entre variantes e haplótipos do gene IL10 com a infecção por HPV em uma coorte da população brasileira. Nossos achados sugerem que variantes genéticas do gene IL10 podem influenciar a suscetibilidade à infecção por HPV, apontando os haplótipos como potenciais biomarcadores de risco. | dc.description.abstractother1 | Interleukin 10 (IL-10) is an anti-inflammatory and immunosuppressive cytokine that regulates the inflammatory response and cellular immunity against viral infections, including Human Papillomavirus (HPV). Changes in IL-10 expression and secretion, potentially caused by single nucleotide variants (SNVs) in the promoter region of the IL10 gene, may influence viral persistence and the progression to squamous intraepithelial lesions and cervical cancer. This study aimed to evaluate the association of alleles, genotypes, and haplotypes of the SNVs rs1800896 (-1082 A>G), rs1800871 (-819 C>T), and rs1800872 (-592 C>A) in the IL10 gene with HPV infection, the development of squamous intraepithelial lesions, and cervical cancer in women receiving care in public health services in Paraná, Brazil. This is a case-control study approved by the Human Research Ethics Committee of the State University of Londrina (CAAE 38937520.2.0000.5231). A total of 438 women were included in the study, grouped according to the presence (n=257) or absence (n=181) of HPV infection. The infected group was further classified based on the severity of cervical lesions: no lesion (n=91), low-grade squamous intraepithelial lesion (LSIL; n=25), high-grade squamous intraepithelial lesion (HSIL; n=72), and cervical cancer (n=69). Peripheral blood samples were collected for IL10 genotyping by Real-Time PCR, and cervical samples were used for HPV detection by conventional PCR. Statistical analyses were performed using SPSS version 25.0 (p<0.05), and haplotype inference was conducted using the PHASE software. The results showed that the G allele of rs1800896 was associated with a protective effect against HPV infection in the dominant model (AA vs AG+GG; OR = 0.646, 95% CI: 0.439–0.950; p = 0.026). In contrast, the T allele of rs1800871 in the dominant model (CC vs CT+TT) was associated with increased susceptibility (OR = 2.056, 95% CI: 1.388–3.045; p < 0.001), as were the CA+AA genotypes of rs1800872 (OR = 2.056, 95% CI: 1.388–3.045; p < 0.001). Haplotype analysis indicated that the ATA haplotype was significantly associated with HPV infection both in homozygous individuals (ATA/ATA: OR = 2.238, 95% CI: 1.155–4.339; p = 0.017) and in combination with other haplotypes (ATA/others: OR = 1.807, 95% CI: 1.202–2.718; p = 0.004). In contrast, the GCC haplotype was associated with a protective effect against HPV infection in heterozygous carriers (GCC/others: OR = 0.628, 95% CI: 0.420–0.940; p = 0.024). However, after adjustment for confounding factors in logistic regression models, these associations with lesions and cancer were no longer statistically significant (p > 0.05). This is the first study to evaluate the association between IL10 gene variants and haplotypes with HPV infection in a Brazilian population cohort. Our findings suggest that IL10 genetic variants may influence susceptibility to HPV infection, indicating haplotypes as potential risk biomarkers.
