Associação entre as variantes genéticas NLRP3 T>C (rs10157379), NLRP3 C>G (rs10754558), IL18 C>G (rs187238) e IL18 G>A (rs360717) com suscetibilidade, acometimento articular e gravidade da psoríase

| dc.description.abstract | INTRODUÇÃO: A psoríase (PsO) é uma doença inflamatória crônica, imunomediada, multifatorial e poligênica que afeta aproximadamente 2,0% da população mundial. Pode se manifestar com artrite psoriásica (PsA) e está frequentemente associada a comorbidades metabólicas e cardiovasculares. Sua fisiopatologia envolve a ativação do inflamassoma NLRP3, responsável pela produção da interleucina (IL)-1ß e IL-18, e estimulação do eixo IL-23/Th17, que impulsiona o processo inflamatório. Componentes genéticos como as variantes de nucleotídeos únicos (SNVs) de genes das vias inflamatórias envolvidas podem determinar aumento do risco, assim como, fenótipos variados da doença. O papel das variantes NLRP3 C>G (rs10754558) e IL18 C>G (rs187238) na fisiopatologia da PsO e da PsA é inconsistente e escasso e, até onde sabemos, não existem dados disponíveis sobre as variantes NLRP3 T>C (rs10157379) e IL18 G>A (rs360717) em indivíduos com PsO. Ao particularizar-se a população brasileira, cuja heterogeneidade genética é marcante, esse hiato de informações revela-se ainda mais importante. OBJETIVOS: Investigar a associação das variantes genéticas NLRP3 T>C (rs10157379) e NLRP3 C>G (rs10754558) com suscetibilidade, envolvimento articular e gravidade da PsO; assim como das variantes IL18 C>G (rs187238) e IL18 G>A (rs360717) com suscetibilidade e gravidade da PsO. METODOLOGIA: Foi realizado um estudo caso-controle com 256 indivíduos (128 pacientes com PsO e 128 controles saudáveis), com idades entre 18 e 69 anos. Os pacientes foram acompanhados no Ambulatório de Dermatologia do Hospital Universitário de Londrina, e os controles foram recrutados do Hemocentro Regional de Londrina. A gravidade da doença foi avaliada utilizando o Índice de Área e Gravidade de Psoríase (PASI). As SNVs de NLRP3 e de IL18 foram determinadas por ensaio de discriminação alélica utilizando a reação em cadeia da polimerase em tempo real. RESULTADOS: Os resultados foram descritos e discutidos em um artigo original (Artigo 1) e uma Carta ao Editor. O Artigo 1 demonstra que não foram observadas associações significativas entre as variantes do NLRP3 com suscetibilidade ou gravidade da PsO. No entanto, para a variante NLRP3 T>C (rs10754558), o genótipo CG foi associado a menor chance de PsA [odds ratio (OR): 0,32; intervalo de confiança (IC) 95%: 0,10–0,95; p=0,041], enquanto o genótipo GG conferiu maior chance (OR: 3,26; IC 95%: 1,03–10,32; p=0,044), sugerindo um efeito protetor do genótipo heterozigoto desta variante para PsA, possivelmente relacionado à modulação intermediária da atividade do inflamassoma e da produção de IL-1ß/IL 18. Não foi observada associação entre a variante NLRP3 T>C (rs10157379) e PsA. A Carta ao Editor relata que não foram observadas associações significativas entre as variantes do IL18 com suscetibilidade ou gravidade da PsO. CONCLUSÃO: As variantes NLRP3 T>C (rs10157379), NLRP3 C>G (rs10754558), IL18 C>G (rs187238) e IL18 G>A (rs360717) não foram associadas à suscetibilidade ou à gravidade da PsO nesta coorte brasileira. No entanto, quando analisada a partir da recíproca do OR, a comparação em relação ao genótipo CG da variante NLRP3 C>G (rs10754558) indicou que o genótipo de referência CC apresenta aproximadamente 3,1 vezes mais chance de desenvolver a PsA. Esse achado reforça o possível efeito protetor do genótipo CG em relação ao envolvimento articular. De forma complementar, ao considerar a recíproca do OR para o genótipo GG da mesma variante, observa-se que o genótipo de referência CC apresenta cerca de 69,0% menor chance de PsA. | dc.description.abstractother1 | INTRODUCTION: Psoriasis (PsO) is a chronic, immune-mediated, multifactorial, and polygenic inflammatory disease that affects approximately 2.0% of the global population. It may present with psoriatic arthritis (PsA) and is frequently associated with metabolic and cardiovascular comorbidities. Its pathophysiology involves activation of the NLRP3 inflammasome, responsible for the production of interleukin (IL)-1ß and IL-18, and stimulation of the IL-23/Th17 axis, which drives the inflammatory process. Genetic components such as single nucleotide variants (SNVs) in genes of the involved inflammatory pathways may increase disease risk and contribute to its phenotypic variability. The role of NLRP3 C>G (rs10754558) and IL18 C>G (rs187238) variants in the pathophysiology of PsO and PsA is inconsistent and scarce, and to our knowledge, there are no data available on the NLRP3 T>C (rs10157379) and IL18 G>A (rs360717) variants in individuals with PsO. When considering the Brazilian population, whose genetic heterogeneity is marked, this information gap becomes even more significant. OBJECTIVES: To investigate the association of the genetic variants NLRP3 T>C (rs10157379) and NLRP3 C>G (rs10754558) with susceptibility, joint involvement, and PsO severity, as well as the association of the IL18 C>G (rs187238) and IL18 G>A (rs360717) variants with susceptibility to and severity of PsO. METHODOLOGY: A case–control study was conducted including 256 individuals (128 patients with PsO and 128 healthy controls), aged between 18 and 69 years. Patients were followed at the Dermatology Outpatient Clinic of the University Hospital of Londrina, and controls were recruited from the Regional Blood Center of Londrina. Disease severity was assessed using the Psoriasis Area and Severity Index (PASI). SNVs of NLRP3 and IL18 were determined by allelic discrimination assays using real-time polymerase chain reaction. RESULTS: The results were described and discussed in an original article (Article 1) and a Letter to the Editor. Article 1 demonstrates no significant associations were observed between the NLRP3 variants and susceptibility to or severity of PsO. However, for the NLRP3 C>G (rs10754558) variant, the CG genotype was associated with a lower likelihood of PsA [odds ratio (OR): 0.32; 95% confidence interval (CI): 0.10–0.95; p = 0.041], whereas the GG genotype conferred a higher likelihood (OR: 3.26; 95% CI: 1.03–10.32; p = 0.044), suggesting a heterozygous protective effect of this variant for PsA, possibly related to intermediate modulation of inflammasome activity and IL-1ß/IL-18 production. No association was observed between the NLRP3 T>C (rs10157379) variant and PsA. The Letter to the Editor reports that no significant associations were observed between the IL18 variants and susceptibility to or severity of PsO. CONCLUSION: The NLRP3 T>C (rs10157379), NLRP3 C>G (rs10754558), IL18 C>G (rs187238) and IL18 G>A (rs360717) variants were not associated with susceptibility to or severity of PsO in this Brazilian cohort. However, when analyzed using the reciprocal of the OR, the comparison relative to the CG genotype of the NLRP3 rs10754558 variant indicated that the reference genotype has approximately 3.1 times greater likelihood of developing PsA. This finding reinforces the potential protective effect of the CG genotype with respect to joint involvement. Complementarily, when considering the reciprocal OR for the GG genotype of the same variant, the reference genotype shows approximately 69% lower likelihood of PsA. Taken together, these findings suggest that although NLRP3 C>G (rs10754558) may not be a primary determinant of PsO onset, it may contribute to disease heterogeneity and to specific clinical phenotypes, such as PsA | |
