Comportamento sedentário em indivíduos com DPOC: pontos de corte preditores de mortalidade a longo prazo e perfil de pacientes respondedores a um programa de reabilitação voltado a reduzir o sedentarismo

dc.description.abstractIntrodução: Apesar de haver diferentes pontos de corte para identificar sedentarismo em indivíduos com DPOC, não se sabe quais deles predizem de fato a mortalidade a longo prazo nessa população. Além disso, a redução do tempo sedentário (TS) é um alvo clínico importante para indivíduos com DPOC pois trata-se de um desfecho modificável com potencial benefícios; no entanto, não se conhece o perfil de pacientes que respondem positivamente a um programa de reabilitação pulmonar (RP) voltado especificamente para redução do comportamento sedentário. Portanto, a presente tese de doutorado foi desenvolvida com dois objetivos principais: 1) identificar a utilidade e comparar três pontos de corte de comportamento sedentário (i.e., TS >8,5 horas/dia; >70% do tempo acordado gasto em comportamento sedentário; e <4300 passos/dia) como preditores de mortalidade por todas as causas em um seguimento de 12 anos em indivíduos com DPOC; e 2) identificar as características de pacientes que respondem positivamente a um programa multidimensional de RP voltado especificamente à redução do TS. Métodos: Com base nesses objetivos, dois estudos originais foram conduzidos. O primeiro investigou a associação dos três diferentes pontos de corte com a mortalidade em um follow-up de 12 anos em 92 indivíduos com DPOC estável. Já o segundo estudo incluiu 22 indivíduos que tiveram seu nível de sedentarismo e atividade física avaliado antes e após a realização de um programa multidimensional de RP especificamente focado na redução do TS, composto por três componentes principais: i) seções educativas com estratégias de mudança de comportamento; ii) uso de um dispositivo tecnológico portátil (“wearable”) que emitia um alerta vibrotátil e alertava o indivíduo a levantar-se e quebrar blocos de sedentarismo; e iii) treinamento físico de alta intensidade incluindo exercício aeróbico e fortaleciemento muscular. Para análise, os indivíduos foram classificados como respondedores ou não respondedores de acordo com a redução (ou não) do TS após o programa de RP. Resultados: No estudo 1, após a análise de curva ROC dos três pontos de corte, foram encontrados os seguintes valores: TS >8,5 horas/dia, área sob a curva (AUC) 0,632 (95% IC 0,510-0,754; P= 0,03), sensibilidade 41% e especificidade 71%; >70% de tempo acordado, AUC 0,602 (95%IC 0,478-0,727; P=0,10), sensibilidade e especifidade 29% e 81%, respectivamente; <4300 passos/dia, AUC 0,620 (95%IC 0,499-0,742; P=0,05), sensibilidade 58% e especificidade 65%. Porém quando comparados os valores de curva ROC, não foram encontradas diferenças significativas. Por outro lado, o ponto de corte TS >8,5 horas/dia foi um preditor independente de mortalidade (hazard ratio 1,23; 95%IC 1,021–1,589; P=0,02), ao contrário dos outros dois pontos de corte. No segundo estudo, quando os indivíduos foram classificados como respondedores (n=12) ou não-respondedores (n=10) com base na redução (ou não) do TS/dia pós-RP, a mudança pós-pré no TS/dia foi de -97±95min/dia e 86±82min/dia, respectivamente (P<0,001). Respondedores apresentavam maior TS/dia e menor tempo/dia em atividade física (AF) leve e número de passos/dia na avaliação pré-RP. A respeito das mudanças pós-pré RP, houve diferenças significativas entre respondedores e não-respondedores em tempo/dia em AF leve (26±46 vs -33±57 min/dia) e passos/dia (904 [537–2444] vs -615 [-3575–1288], respectivamente)(P=0,01 para ambos). Diminuição no TS/dia se correlacionou bem com aumento no tempo/dia em AF leve, tanto no grupo geral (r=-0,83) quanto nos respondedores (r=-0,66), mas não com o aumento no tempo/dia em AF moderada-a-vigorosa (AFMV)(r=-0,33 e -0,13, respectivamente). Regressão linear mostrou associação significativa entre mudanças no TS/dia e no tempo/dia em AF leve (R² ajustado=0,643; P<0,0001). Conclusões: Entre diferentes pontos de corte que indicam comportamento sedentário, TS >8,5 horas/dia foi o indicador independente mais forte do risco de mortalidade em um seguimento de 12 anos em indivíduos com DPOC, indicando um risco 23% maior em comparação com aqueles que apresentam TS <8,5 horas/dia. Além disso, indivíduos com DPOC que respondem positivamente a um programa multidimensional de RP focado especificamente em reduzir o TS eram caracterizados por piores níveis basais de AF comparados aos não respondedores, enquanto a diminuição no TS/dia após o programa se associou fortemente ao aumento da AF leve, mas não da AFMV
dc.description.abstractother1Introduction: Although different cut-off points have been proposed to identify sedentary behavior in individuals with COPD, it remains unclear which of these thresholds actually predict long-term mortality in this population. Furthermore, reducing sedentary time (ST) is an important clinical target in COPD, as it represents a modifiable outcome with potential health benefits. However, the profile of patients who respond positively to pulmonary rehabilitation (PR) programs specifically aimed at reducing sedentary behavior (SB) is not well understood. Therefore, this doctoral thesis was developed with two main objectives: (1) to assess the utility of and compare three sedentary behavior cut-off points (i.e., ST >8.5 hours/day; >70% of awake time spent in SB; and <4300 steps/day) as predictors of all-cause mortality over a 12-year follow-up in individuals with COPD; and (2) to identify the characteristics of patients who respond positively to a multidimensional PR program specifically focused on reducing sedentary behavior. Methods: Based on these objectives, two original studies were conducted. The first study investigated the association between the three different sedentary behavior cut-off points and mortality over a 12-year follow-up in 92 individuals with stable COPD. The second study included 22 individuals who had their levels of SB and physical activity assessed before and after participating in a multidimensional PR program specifically designed to reduce ST. The program included three main components: (i) educational sessions incorporating behavior change strategies; (ii) the use of a wearable device that provided vibrotactile alerts prompting the individual to stand up and interrupt prolonged sedentary bouts; and (iii) high-intensity physical training including aerobic exercise and resistance training. For analysis, participants were classified as responders or non-responders based on whether or not they reduced their ST following the PR program. Results: In study 1, after analysis of the ROC curves of the three cutoffs, the following values were found: ST >8.5 hours/day, area under the ROC curve (AUC) 0.632 (95%CI 0.510 – 0.754; P=0.03), sensitivity 41% and specificity 71%; >70% of the awake time, AUC 0.602 (95%CI 0.478 – 0.727; P = 0.10), sensitivity and specificity 29% and 81%, respectively; <4300 steps/day AUC 0.620 (95%CI 0.499 – 0.742; P=0.05), sensitivity 58% and specificity 65%. However, when comparing the ROC curves, no significant differences were found between the three SB cutoffs. On the other hand, the ST >8.5 hours/day threshold was an independent predictor of mortality (hazard ratio 1.23; 95%CI 1.021–1.589; P=0.02), whereas the other two cut-off points were not. In study 2, when individuals were classified as responders (n=12) or non-responders (n=10) based on their change in ST post-PR, the mean pre-post change in ST/day was -97±95 min/day and 86±82 min/day, respectively (P<0.001). At baseline, responders had higher ST/day and lower time/day spent in light physical activity (PA) and steps/day compared to non-responders. Regarding changes following PR, significant differences between groups were observed in time/day spent in light PA (26±46 vs. -33±57 min/day) and steps/day (904 [537–2444] vs. -615 [-3575–1288], respectively) (P=0.01 for both). Reductions in ST/day were strongly correlated with increases in time/day spent in light PA in both the overall group (r=-0.83) and responders (r=-0.66), but not with changes in moderate-to-vigorous physical activity (MVPA) (r =-0.33 and -0.13, respectively). Linear regression showed a significant association between changes in ST/day and light PA/day (adjusted R² = 0.643; P<0.0001). Conclusions: Among the different cut-off points used to define sedentary behavior, ST >8.5 hours/day emerged as the strongest independent predictor of mortality over 12 years in individuals with COPD, indicating a 23% higher risk compared to those with ST <8.5 hours/day. Moreover, individuals with COPD who responded positively to a multidimensional PR program specifically targeting sedentary behavior were characterized by worse baseline levels of physical activity compared to non-responders, and the reduction in ST/day was strongly associated with increase in light PA, but not in MVPA
