Interação das variantes do gene APOE rs429358 e rs7412 com a aptidão física, biomarcadores metabólicos, fragilidade, e treinamento físico em mulheres idosas

dc.description.abstractIntrodução: O exercício físico pode modular o perfil lipídico, os componentes da aptidão física e o nível de fragilidade no processo de envelhecimento e esse efeito parece ser influenciado pelas variantes do gene APOE. Objetivos: Analisar e comparar o perfil lipídico, aptidão física e fragilidade em mulheres idosas categorizadas segundo os haplótipos (rs429358 e rs7412) do gene APOE (Estudo 1) e investigar se os grupos respondem diferentemente ao treinamento físico multicomponente, em relação aos marcadores metabólicos, aptidão física e fragilidade (Estudo 2). Métodos: 114 Mulheres com = 60 anos foram classificadas em 3 grupos de acordo com as variantes do gene APOE: E2, portadoras do alelo ?2 (?1?2; ?2?2; ?2?3; n=13); E3, homozigotas para o alelo ?3 (?3?3; n=79), e E4, portadoras do alelo ?4 (?3?4; ?4?4; n=22). Uma subamostra do estudo (n=72) foi submetida a exercícios por 12 semanas e às avaliações pós-intervenção. Foram realizadas avaliação do perfil lipídico (CHOL, HDL-c, LDL-c) por autoanalizador bioquímico, do nível de fragilidade (FRAG) pela Escala de Fragilidade de Edmonton, do desempenho físico pelo Short Physical Performance Battery (SPPB), que envolve equilíbrio (EQUI), velocidade da marcha (VELM) e força e resistência de membros inferiores, além de realização do Teste de Agilidade e Equilíbrio Dinâmico (AGI), Caminhada de 6 minutos (TC6) e preensão manual (PMAN), e dinamômetro isocinético para avaliação de força de membros inferiores. Resultados: Os resultados referentes à comparação das idosas destreinadas mostraram diferenças significativas nas variáveis VELM (p = 0,001), SPPB (p= 0,051), AGI (p = 0,018) e na FRAG (p=0,038) onde o grupo E2 apresentou pior desempenho comparado ao grupo E3. As análises de regressão logística, indicaram que idosas que portam o alelo ?4 em relação as que possuem o alelo ?3, apresentam 9,47 vezes mais chances de terem um desempenho fraco/regular no SPPB; 3,97 vezes mais chances de terem desempenho fraco no teste AGI e, 4,05 vezes mais chances de serem classificadas como vulneráveis/frágeis no teste FRAG. Em relação aos efeitos do exercício físico, as comparações inter e intragrupos mostraram que as idosas, independente, das variantes do gene APOE, melhoraram o desempenho em nove das 11 variáveis de aptidão física (p<0,05 para todos). As análises evidenciaram ainda efeito na interação grupo*tempo nas variáveis de potência muscular em que os grupos E2 e E3 apresentaram maior evolução durante o período de treinamento em relação ao grupo E4 (PKTOQ180ºEX; E2 46,3±9,2 vs. 54,1±12,9; E3= 53,8±13,4 vs. 58,7,±16; p=0,022 e PKTOQ180ºFLEX; E2= 26,7±10,1 vs. 33,6,±10; E3= 33,9±8,9 vs. 38,2,±9,0, p=0,006). Conclusão: A manifestação de diferentes respostas, de acordo com as variantes gene APOE em mulheres idosas, reforça a importância da genética na personalização de intervenções que sejam adaptadas à heterogeneidade biológica do processo de envelhecimento.
dc.description.abstractother1Introduction: Physical exercise can modulate the lipid profile, physical fitness components and the level of frailty in the aging process and this effect appears to be influenced by APOE gene variants. Objectives: To analyze and compare the lipid profile, physical fitness and frailty in elderly women categorized according to the haplotypes (rs429358 and rs7412) of the APOE gene (Study 1) and investigate whether the groups respond differently to multicomponent physical training, in relation to metabolic markers, physical fitness and frailty (Study 2). Methods: 114 Women aged = 60 years were classified into 3 groups according to APOE gene variants: E2, carriers of the ?2 allele (?1?2; ?2?2; ?2?3; n=13); E3, homozygous for the ?3 allele (?3?3; n=79), and E4, carriers of the ?4 allele (?3?4; ?4?4; n=22). A subsample of the study (n=72) underwent exercise for 12 weeks and post-intervention assessments. Assessment of the lipid profile (CHOL, HDL-c, LDL-c) was carried out using a biochemical autoanalyzer, the level of frailty (FRAG) using the Edmonton Frailty Scale, physical performance using the Short Physical Performance Battery (SPPB), which involves balance (EQUI), gait speed (VELM) and strength and resistance of the lower limbs, in addition to carrying out the Agility and Dynamic Balance Test (AGI), 6-minute walk (TC6) and hand grip (PMAN), and isokinetic dynamometer for lower limb strength assessment. Results: The results regarding the comparison of untrained elderly women showed significant differences in the variables VELM (p = 0.001), SPPB (p = 0.051), AGI (p = 0.018) and FRAG (p = 0.038) where the E2 group presented worse performance compared to group E3. Logistic regression analyzes indicated that elderly women who carry the ?4 allele compared to those who have the ?3 allele are 9.47 times more likely to have poor/regular performance on the SPPB; 3.97 times more likely to have poor performance in the AGI test and 4.05 times more likely to be classified as vulnerable/frail in the FRAG test. Regarding the effects of physical exercise, inter and intragroup comparisons showed that the elderly women, regardless of the APOE gene variants, improved performance in nine of the 11 physical fitness variables (p<0.05 for all). The analyzes also showed an effect on the group*time interaction on the muscle power variables in which the E2 and E3 groups showed greater evolution during the training period in relation to the E4 group (PKTOQ180ºEX; E2 46.3±9.2 vs. 54, 1±12.9; E3= 53.8±13.4 vs. 58.7.±16; 33.9±8.9 vs. 38.2±9.0, p=0.006). Conclusion: The manifestation of different responses, according to APOE gene variants in elderly women, reinforces the importance of genetics in personalizing interventions that are adapted to the biological heterogeneity of the aging process.
