| dc.description.abstract | Resumo: Introdução: A Síndrome da Dor no Grande Trocânter (SDGT) é causa comum de dor lateral no quadril em mulheres de meia-idade ativas, e está associada às alterações biomecânicas angulares na pelve e quadril. Porém, não existem relatos sobre alterações cinemáticas de todo o membro inferior na realização de testes funcionais e na marcha. Objetivo: Investigar possíveis diferenças angulares da pelve, quadril, joelho e tornozelo em tarefas funcionais como descida de degrau e marcha em mulheres com e sem SDGT. Materiais e Métodos: A presente tese está dividida em dois estudos. No estudo 1 foram avaliadas 32 mulheres distribuídas em grupo SDGT (n=16) e grupo controle (n=16), e o estudo 2 foi composto por 24 mulheres distribuídas em grupo SDGT (n=12) e grupo controle (n=12). Todas as participantes preencheram o questionário de caracterização da amostra, a escala numérica de dor e o VISA Tendinopathy Questionnaire for Greater Trochanteric Pain Syndrome (VISA-G). Posteriormente, foram submetidas à análise cinemática do membro inferior durante os testes Lateral e Forward Step-Down (estudo 1) e durante a marcha (estudo 2). A captura de movimento foi realizada com um sistema OptiTrack® de oito câmeras (120 Hz), e marcadores retrorrefletivos foram posicionados em pontos de referência e grupos anatômicos. Os dados angulares da pelve, quadril, joelho e tornozelo foram calculados usando Visual3D e MATLAB. As comparações entre os grupos foram realizadas usando o teste de Mann-Whitney, e a análise temporal dos ângulos articulares por Statistical Parametric Mapping (SPM). Resultados: O estudo 1 apontou que durante os testes Lateral e Forward Step Down não houve diferenças significativas nos valores mínimo e máximo para variáveis angulares nas articulações da pelve, quadril, joelho e tornozelo entre os grupos (P > 0,05). No entanto, a análise por SPM revelou diferenças no desempenho entre os grupos, com maior flexão do quadril (63–87%; P < 0,01), rotação interna (45–55%; P < 0,01), aumento da flexão do joelho (80–100%; P < 0,01) e redução da dorsiflexão do tornozelo (23–100%; P < 0,01) no grupo GTPS. Durante o FSD, mulheres com GTPS apresentaram redução da inclinação pélvica anterior (0–100%; P < 0,01), maior rotação interna do quadril (58–95%; P < 0,01) e menor dorsiflexão do tornozelo (22–100%; P < 0,01). No estudo 2 a análise por SPM durante a marcha demonstrou que mulheres com SDGT apresentaram maior queda pélvica entre 1–12% e 81–100% do ciclo da marcha, e maior rotação interna do quadril entre 86–98%. Diferenças no joelho foram observadas, com maior flexão entre 72–89%, maior varo entre 14–67% e 85–100%, e maior rotação externa entre 42–54% e 87–99%. Conclusão: Mulheres com SDGT apresentaram alterações cinemáticas na pelve, quadril, joelho e tornozelo durante os testes em step. | dc.description.abstractother1 | Abstract: Introduction: Greater Trochanteric Pain Syndrome (GTPS) is a common cause of lateral hip pain in active middle-aged women and is associated with angular biomechanical changes in the pelvis and hip. However, there are no reports on kinematic changes throughout the lower limb during functional tests and gait. Objective: To investigate potential angular differences of the pelvis, hip, knee, and ankle in functional tasks such as stair descent and gait in women with and without GTPS. Methods: Article 1 included a sample of 32 women allocated to a GTPS group (n = 16) and a control group (n = 16). Article 2 included 24 women allocated to a GTPS group (n = 12) and a control group (n = 12). Participants completed a sample characterization questionnaire, the Numerical Pain Rating Scale, and the VISA Tendinopathy Questionnaire for Greater Trochanteric Pain Syndrome (VISA-G). Subsequently, they performed lower-limb kinematic analysis during the Lateral and Forward Step-Down tests and during gait. Motion capture was performed using an eight-camera OptiTrack® system (120 Hz), and retroreflective markers were placed on anatomical landmarks and clusters. Pelvic, hip, knee, and ankle angular data were calculated using Visual3D and MATLAB. For the step tests, participants performed six repetitions per task, and four were used for analysis. For gait, five trials were performed at self-selected speed along a 10-meter walkway. Between-group comparisons were performed using the Mann–Whitney test, and temporal analysis of joint angles was conducted using Statistical Parametric Mapping (SPM). Results: In the Lateral and Forward Step-Down tests, no significant differences were observed in minimum and maximum angular values of the pelvis, hip, knee, and ankle between groups (P > 0.05). However, SPM analysis revealed performance differences, with greater hip flexion (63–87%; P < 0.01), greater hip internal rotation (45–55%; P < 0.01), increased knee flexion (80–100%; P < 0.01), and reduced ankle dorsiflexion (23–100%; P < 0.01) in the GTPS group during LSD. During FSD, women with GTPS showed reduced anterior pelvic tilt (0–100; P < 0.01), greater hip internal rotation (58–95%; P < 0.01), and lower ankle dorsiflexion (22–100%; P < 0.01). Regarding gait, SPM demonstrated that women with GTPS presented greater pelvic drop between 1–12% and 81–100% of the gait cycle and greater hip internal rotation between 86–98%. Knee differences were also observed, with greater flexion between 72–89%, greater varus between 14–67% and 85–100%, and greater external rotation between 42–54% and 87–99%. Conclusion: Women with GTPS exhibit reduced pelvic motion and greater hip internal rotation amplitude, as well as decreased ankle motion during step-down tests.
